🌻Appts available: Monday - Saturday; at 11 am, 1pm, 3pm, 5 pm and if you have an earlier or later time in mind, please let us know so we can try to accommodate you.

Appts are targeted to be within a 2 hr. window (unpredictable travel and appt time). For ex: if you have a 1 pm appt with us, arrival may be between 12:30 to 1:30.

🌷After you request an appt, you'll receive an automated email followed by a TEXT and EMAIL with availability and costs. If we do not respond within 1 hour (during business hrs.), please text us at: 360-524-3328. Due to privacy while helping other families, to keep costs less than other providers, and traveling, we are unable to answer phone calls or frequently monitor voicemails.

PREFERENCE FOR AFTERCARE? Communal/Individual cremation/ Home Burial. ________(Individual Cremation package will be delivered to your home within 7-14 days after appt via signed delivery. Your presence is required for delivery. An email from UPS to you, tends to be sent the day before delivery).

I agree to the following:

NO aggressive or reactive dogs over 20 lbs. This includes any of the following behaviors, including but not limited to: growling, evasive, unable to pet, hold still, give injections to, etc. If any person or pet in the home makes our staff feel uncomfortable or unsafe in any way, we have the right to not schedule an appt, cancel, or discontinue an appt at any time. ____If any other pet(s) in the home have ANY potential for aggression or disturbing us during the appointment, they will be secured safely away from our team or services will not be rendered.

If I choose cremation/aftercare for my pet over 20 lbs, there will be at least one person at my home available to help carry my pet to the car unless otherwise arranged and confirmed with us. If my pet is over 50 lbs, there will be at least 2 people at my home to help carry my pet to the car. Dr. Cory has a medical weight (torn rotater both shoulders)limit over 20 lbs by herself.

I will disclose if anyone in our household is ill with anything potentially contagious, so our team can be prepared. We provide our own masks, gloves, and sanitizer. Thank you.
